The 34-year-old Portugal international did not join the rest of Jose Mourinho's squad as they flew out to the United States.
The La Liga champions take on David Beckham's Los Angeles Galaxy on Friday in their opening pre-season match but Calvalho will miss out, prompting fresh doubt over his future at the Bernabeu.
Spurs boss Andre Villas Boas could now be encouraged to rekindle his interest in the centre-half to vill the void of experience left by Ledley King, who will act as a club ambassador next season following his retirement.
Villas Boas already has five central defenders in Jan Vertonghen, Steven Caulker, Younes Kaboul and Michael Dawson to choose from but Frenchman William Gallas expected to leave ahead of the new season.

Real were hopeful of including Carvalho as part of their offer to sign Luka Modric. But Spurs are holding out for £40million for the Croatian playmaker.
Mourinho, however, did include Kaka and Nuri Sahin in Madrid's touring party despite uncertainty over their futures.
Brazilian star Kaka has been linked with a return to AC Milan while Arsenal are believed to closing in on a loan deal for midfielder Sahin.
